Forms for Clubs, Student Organizations, and BSG Programs

Program Registration - Student organizations and programs must complete this form in order to hold events and host long-term programs on the Davie, Fort Lauderdale, or Dania Beach campuses.

Clubs and Student Organizations: Complete Once

New Clubs and Organizations - Brand new student organizations must complete these forms and submit them to the Office of Student Involvement and Leadership in order to be recognized by Broward Student Government and its Council of Student Organizations.

Clubs and Student Organizations: Complete Every Semester

Club Status Report Form - Clubs and student organizations must complete this form every semester to remain active and "in status."

Clubs and Student Organizations: Complete As Necessary

Club Authorized Signature Form - When your officers change, we need to have their signatures on file for fiscal purposes.

COSO Funding Request - Student organizations can request to use funding provided by the Council of Student Organizations.

Travel Registration
- Funding for travel is available on a limited basis. Please call the Office of Student Involvement and Leadership at 954-236-1219 for more information.
