- Event Proposal - This form must be filled out to get approval prior to planning an event.
- Program Registration - THis form is to register events with the Office of Student Involvement and Leadership once the Event Proposal has been approved.
- Expense Justification Form - This form is used to justify expenditures.
- Volunteer Log - This form allows you to record your hours of service to the community on your official FAU transcripts.
- New Student Organization Formation Packet - This packet must be filled out prior to starting a new student organization.
