FAU MARCHING OWLS – Medical Forms
To participate in the Marching Owls ensemble-class, the following medical documentation is required of all participating students:
- Medical History (form completed by the student and brought to preseason training)
- Health Insurance Information (form completed by the student and brought to preseason training.
Medical History:
This four-page document provides information about your
health that is used by the Marching Owls staff in case of an
emergency.
Insurance
Information:
This one-page document provides information about your
health insurance that is used by the Marching Owls staff in case
of an emergency. Having this information available will reduce
the chances of delay in providing care during an emergency
situation.
Confidentiality:
All of the medical and insurance information required for
participation in Marching Owls is for use by FAU Department of
Music personnel only. All information will be kept strictly
confidential. Every effort will be made to protect your privacy,
but we must have this information in case of an emergency. Thanks
