All Dive Plans submitted to the DSO MUST have the Diving Accident Emergency Management Plan completed containing emergency contact information, nearest medical facility, and nearest recompression chamber.

Phone numbers, addresses and contact names provided below are intended to be a general guide and were revised for accuracy on 6/25/07.  It is the responsibility of the Lead Diver to assure that all contact information submitted on the Dive Plan to the DSO is complete and accurate.
